HealthySteps

HealthySteps is an evidence-based, interdisciplinary pediatric primary care program designed to promote nurturing parenting and healthy development for babies and toddlers.  The HealthySteps (HS) approach means the entire practice works as a team to implement 8 core components to strengthen the relationships between families and the practice and support strong parent/child attachment.

The HealthySteps model is organized into three tiers of service.

Universal services include:

  • Child developmental, social-emotional and behavioral screening
  • Screening for family needs (i.e., maternal depression, other risk factors, social determinants of health)
  • Child development support line (e.g., phone, text, email, online portal)

Short-term supports includes tier one services plus:

  • Child development and behavior consults
  • Care coordination and systems navigation
  • Positive parenting guidance and information
  • Early learning resources

Comprehensive services includes all tier one and two services plus:

  • Ongoing, preventative team-based well-child visits (WCV)
