MyChart Information

MyChart is a secure website or app where people can access their health information, such as test results, vaccines, and visit notes.

 

What is a medical proxy and MyChart proxy access?

A medical proxy is someone allowed to help another person with their healthcare. This person can view health records, schedule appointments, send messages to the doctor, and help manage care.

 

MyChart proxy access gives legal guardians, parents, and adults access to a patient’s medical record. Proxy access can be given regardless of patient MyChart status; for example, patients’ MyChart may state Active, Inactive, Active (Only Proxy), Inactive (user deactivated), etc. Several types of proxy access can be granted to a patient’s MyChart account. A proxy will either have full or limited access.

 

Here’s how proxy access works based on age:

Ages 0–11

  • Parents or guardians have full access
  • They can see all health information and help with appointments and messages
  • To be a proxy, the adult must be listed in the child’s medical records

Ages 12–14

  • Parents or guardians can still have full access if both patient and parent fill out a Primary Access Application
  • If the teen wants to create their own account, the parent/guardian can:
    • view immunizations
    • send messages to the care team
    • view family history
  • At this age, some health information may be kept private to support the teen’s growing independence

Ages 15–17

  • Parents and guardians can still be a proxy, but some parts of the teen’s health information may be private to follow privacy laws
  • Certain messages and appointment details may not be visible to the proxy
  • If someone who is not the parent or legal guardian needs access, a People’s MyChart Proxy Application must be filled out

Age 18 and older

  • When people turn 18, they are considered adults and manage their own MyChart account.
  • An adult patient can grant a spouse, family member, or caregiver access to their health information by filling out a People’s MyChart Proxy Application
  • The proxy will be granted full proxy access

Special Cases

Diminished Capacity

If an adult cannot make health decisions on their own (this is called diminished capacity), a trusted person can be assigned as a proxy.

Foster Parents

Foster parents may be given proxy access for children and teens in their care.

Set Up Two-Step Verification

All MyChart accounts require two-factor authentication (2FA) to access a new MyChart account or access MyChart from an unsaved device. Two-factor authentication requires an email address, a telephone number that can receive calls or a mobile number that can receive text messages to receive the additional code needed to access MyChart accounts.

 

Starting May 19, 2025, everyone must use two-step verification to keep their MyChart account safe. When you log in from a new device, you’ll be asked for a special code after your password. It helps make sure only you can get into your account.
