MyChart is a secure website or app that allows individuals to access their health information, including test results, vaccination records, and visit notes.
What Is a Medical Proxy and MyChart Proxy Access?
A medical proxy is someone you trust to help manage your healthcare. This person can view your medical records, schedule appointments, and send messages to your doctor through MyChart.
State-Mandated Full Proxy Access for Minors
Under Texas law (House Bill 4224), parents or legal guardians have full access to their child’s medical records in MyChart until the child turns 18. After that, access is removed. If the patient wants someone to continue having access, they must complete and sign the MyChart Full Proxy Access application on their behalf.
Effective September 1, 2025, Texas Senate Bill 1188 requires that health care providers give parents (and managing conservators or guardians) complete and unrestricted access to a minor’s electronic health record immediately, unless access to all or part of the record is restricted under state or federal law or by a court order.
Can Someone Other Than a Parent Be a Proxy for a Minor?
If the patient is between 12 and 17 years old, a trusted adult (other than the patient’s parent or guardian) can be granted full medical proxy access if both the patient and their parent or guardian complete and sign the MyChart Full Proxy Access application.
Can Adults Have a Medical Proxy?
Yes. If you’re 18 or older, you can give another adult access to your MyChart account by completing the MyChart Full Proxy Access application online or during a clinic visit.
Here’s how proxy access works based on age:
Ages 0–17
- Parents or guardians have full access
- You must be a parent or guardian to get full access
- They can see all health information and help with appointments and messages
- To be a proxy, the adult must be listed in the child’s medical records
Ages 12–17

Parents or guardians still have full access
- A trusted adult (other than the patient’s parent or guardian) can be granted full medical proxy access if both the patient and their parent or guardian complete and sign the MyChart Full Proxy Access application
Age 18 and older
- When a patient turns 18, any proxy access anyone has to them, will be automatically terminated
- An adult patient can grant a spouse, family member, or caregiver access to their health information by filling out a People’s MyChart Proxy Application
- The proxy will be granted full proxy access
Special Cases
Diminished Capacity
If an adult is unable to make health decisions on their own (this is called diminished capacity), a trusted person can be appointed as a proxy.
Foster Parents
Foster parents may be given full proxy access for children and teens in their care.



Set Up Two-Step Verification
All MyChart accounts require two-factor authentication (2FA) to access a new MyChart account or access MyChart from an unsaved device. Two-factor authentication requires an email address, a telephone number that can receive calls or a mobile number that can receive text messages to receive the additional code needed to access MyChart accounts.
Starting May 19, 2025, everyone must use two-step verification to keep their MyChart account safe. When you log in from a new device, you’ll be asked for a special code after your password. It helps make sure only you can get into your account.
