What to Bring for Financial Assistance

People’s Community Clinic (People’s) is a Federally Qualified Health Center (FQHC) dedicated to providing high-quality healthcare to all individuals, regardless of their ability to pay.

In accordance with federal guidelines, People’s will not deny services based on a patient’s inability to pay. While we are committed to making care accessible, we are NOT a free clinic.

You are not required to accept specific services to participate in, or receive assistance from, any program at People’s. All services are provided voluntarily.

People’s offers a sliding fee scale discount program based on your income and family size.

To apply, you must meet with a Financial Counselor and provide current financial documents. It is your responsibility to make sure the information is correct and up to date. If you choose not to apply for the program, People’s respects your decision, and you will be responsible for the full cost of your care at People’s. During your financial screening, you will also be screened for potential eligibility to healthcare programs. To provide this service, please bring the items listed in the document below.

You are responsible for providing correct and up-to-date insurance information at the time of service, if applicable. If you have active medical or dental insurance, we will bill the insurance provider accordingly. Please provide all the medical insurances you may have. Failure to provide any and all insurance may result in denials by your insurance plan. You are responsible for any co-pays, deductibles, non-covered, or out-of-network services.

 

You are financially responsible for all charges incurred for services rendered by People’s, regardless of insurance status. Any amount not covered by insurance or sliding scale adjustments will be billed to you or the person legally responsible.

 

You are responsible for informing the clinic of any changes to insurance coverage, address, family size, or income status. If you qualify for the discount program, you are responsible for reapplying annually or if your income or household size changes.

We may be able to waive certain fees based on your situation. Please ask a front desk team member for assistance with a fee waiver request.
